SHAPES (Salem Hospital Ambulatory Parenteral and Enteral Services) is an outpatient clinic providing a variety of services to patients in Salem and surrounding communities, including infusion, wound care, anticoagulation management, and travel consultation and immunization services. Patients are referred to the Salem SHAPES clinic by physicians and other healthcare providers.
Scope of Patient Care
Department Duties
- Patient care planning is interdisciplinary.
- The Plan of Care is initiated and directed by Registered Nurses, who coordinate services and collaborate with the SHAPES Pharmacy, patients, patient caregivers, physicians, and others in providing care.
- Patient care and treatments are administered by Registered Nurses, with support from Certified Nurse Assistants (CNAs) and Certified Medical Assistants (CMAs) who work under the direct supervision of nursing staff.
- Nursing care includes: nursing assessments, reassessments, monitoring, nursing interventions, patient and family education, and discharge planning.
Department Professionals
- Registered Nurses: Licensed by the Oregon State board of Nursing. Wound care nurses are required to have national certification or obtain it within 2 years of date of hire. This certification requires a Bachelor’s Degree in Nursing.
- Certified Nurse Assistants (CNAs): Licensed by the Oregon State Board of Nursing.
- Pharmacists: Licensed by the Oregon Board of Pharmacy. Possess a Bachelor of Science, Master of Science, or Doctor of Pharmacy degree from an accredited college of pharmacy.
- Technical, Clerical, and Other Staff: Completed basic educational requirements and are trained to assist the pharmacists and nurses in activities required to meet patient service requirements. Pharmacy technicians are required to complete on-site training consistent with their duties and must be registered with the Oregon Board of Pharmacy.
