Billing Protocol

Salem Hospital will bill most primary insurance companies. When ordering laboratory testing, please complete all billing and insurance sections on the Test Request form.

When Salem Hospital bills the patient or the insurance company, fees reflect the additional costs involved (Outpatient Fee Schedule).

Occasionally we find it necessary to refer tests to other laboratories. Charges for these tests are subject to change without notice. Handling and transportation charges may be added in addition to the test fee.
