This is the only service of its kind in the Mid-Willamette Valley serving patients in Marion, Polk, Linn, Benton and Lincoln counties and all Portland hospitals.
The staff has many years of training and experience in rehabilitation care.
All of the medical care is provided by a interdisciplinary team. A physician
who specializes in rehabilitation medicine is on site to provide medical care and
direction for the therapy team.
Patient Independence
The staff is dedicated to each patient reaching the highest level of independence possible. |

higher level of independence for activities of daily living (ADLS and IADLs) or daily mobility (walking, wheelchair propulsion). This will
enable the
patient to do more for themselves on a daily basis. |

An improved
quality of life (reducing pain, energy spent for daily mobility and
self-care, return to vocational and leisure/vocational interests,
etc.) |

A safe transition to the community. While working to decrease
the amount of care a patient needs, we simultaneously train appointed
caregivers and family members. While not everyone can return directly
home, we may be
instrumental in getting them to the least restrictive environment. |

Patients will have needs that can be met by two or more
of the following services, to qualify as needing a team approach: |

To Be Considered for Admission
In an effort to serve our region and those patients that who will
benefit from inpatient rehabilitation, the following considerations
are used: |

Need 24 hour rehabilitation nursing. |
| |
Need for a short term rehabilitation stay to achieve return to the community. |
| |
Tolerance of three hours or rehabilitation therapy per day |
| |
Requires at least Physical Therapy and Occupational Therapy to meet rehabilitation goals. |
| |
Have identified at least two places the patient can reside after discharge from rehabilitation. This may include a plan for returning
to the patient’s
previous residence or to an alternative
living setting. |
