Application process & eligibility determination

A request for financial assistance may be made at any point before, during, or after the provision of care.

The hospital has developed an application process for determining initial interest in and qualification for financial assistance. 

Requests for charity assistance will be accepted from the patient directly, or others on the patient’s behalf. 

This could include but is not limited to, the patient’s representative, hospital staff or physicians. 

The hospital will make an initial determination based upon verbal and/or written information received by the patient or the patient’s representative. 

The initial determination can be made at any time prior to payment in full on the account.  Hospital personnel will communicate this initial determination to patients.

A final determination will be made and communicated to the patient upon receipt of a complete financial application (Attachment B) and/or the necessary supporting documentation.
