Consent forms

Mother’s signature, regardless of her age, is required on all consents for the baby. Baby will be identified by mother’s last name while in the hospital.

The following forms require a signature:

• Consent for care for mother and baby.
• You will also be asked to sign the Conditions of Admission (COA). By signing the COA you are agreeing to the financial statement, general hospital care and, unless otherwise requested, you are agreeing to be part of the Patient Directory.
• Consent for procedures (if applicable):
• Epidural anesthesia
• Cesarean section
• Infant circumcision
• If your baby needs special procedures performed you may be asked to sign a consent after the baby’s healthcare provider discusses the procedure with you.
