Protected Health Information (PHI)

When you are admitted to the hospital as a patient, you will be given a personal identification number (PIN).

Share your PIN number only with those persons whom want to receive your health information.

When staff receive calls asking for information about you, and the PIN is correctly provided, the staff may then release the requested information using professional judgment in disclosing the least amount of information necessary.
