Baby’s Birth Certificate

You will be given an application form at the hospital. Within six months from the date of your baby’s birth you will receive a certified copy of the birth certificate when you send the fee and application to the address below.

Cost of the first certified copy of Live Birth – $20

Cost for additional copies if ordered at the  same time – $15

We strongly recommend that you order your child’s birth certificate within the first six months so that corrections by the hospital can be made if necessary.

After the child’s first birthday, the hospital is only able to correct the birth date, time of birth and sex of the baby. All other corrections must go through Oregon Vital Records in Portland and a fee will be charged.

Marion County Vital Statistics may issue certified copies of birth certificates for six months following the date of birth. Any changes to a Paternity Affidavit form, or to the birth certificate, must be sent to the Portland Vital Records office.

After six months from the date of your baby’s birth, you will need to contact the Oregon Center for Health Statistics in Portland to obtain a certified copy of the birth certificate.

The packet given to you at the hospital contains state application forms.
